The most common causes, in order: marine growth on the propeller, a bent or nicked blade, a worn cutless bearing, or engine misalignment. Start with a diver inspection ($100-$150); if the prop is clean and sound, check the bearing and alignment next.
Not always. A diver can remove the propeller in the water and take it to the shop, and engine-shaft alignment is done afloat. Replacing the cutless bearing or shaft seal usually requires a haul-out, so it pays to combine it with your antifouling.
A standard job (straightening blades, repitch, balancing) takes 3-7 days in the shop. Major damage or 5-blade props can take 2 weeks. If you're on a tight schedule before a Canal transit, ask about express service when requesting quotes.
There's no fixed interval: replace it when there's play. Check it at every haul-out: if the shaft moves more than 1-2 mm inside the bearing or vibrates at certain rpm, it's due. With regular use in tropical waters it typically lasts 3-6 years.
